Following is a list of questions and answers that were developed from inquiries by faculty. Faculty and staff should contact the Office of Risk Management, if they have further questions.
Q1: What should faculty (or departments) do with forms once we have students sign them?
A: Warning waiver and release forms are to be maintained in each college.

Q2: What if student won’t sign? Do we develop alternative assignments?
A: If students won’t sign, they should be given alternative assignments.

Q3: What if it is a practicum class, where the course context, by definition, is off-campus. Does failure to sign mean that the student must disenroll?
A: The answer depends on where off-campus. The university has agreements with over 800 sites in the county.

Q4: Is the form required for student teaching enrollment? Or does the district assume liability while the student is there — or in route to the school?
A: We have agreements with every school district in San Diego. If in doubt, contact Office of Risk Management at 619-594-4664.

Q5: Also, does Business and Financial Affairs distribute notices/reminders to faculty/departments each semester as a prompt, or are we supposed to remember to get this done?
A: We distribute this notice about every other year. The colleges are responsible for reminding their faculty of the requirement. The Warning, Waiver and Release of Liability
form can be obtained by calling Office of Risk Management, 619-594-4664 or obtained from the dean’s office.

Q6: If we extend (or begin) student teaching outside of the semester period (due to public school calendars, need for extra time by our students, etc.), are faculty covered by workers’ compensation, university liability insurance, etc. when they go out to supervise the student teachers during their non-contract time?
